Noel was driving a 2000 Dodge Ram pick-up southbound on Oklahoma 2 when he drove off the edge on the west roadway. He came back onto the highway and over-corrected, which overturned the vehicle. The driver and Amber N. Mann were partially ejected from the vehicle, while Teresa E. Milligan was ejected almost 30 feet from the pick-up truck.

Noel, 15, was pronounced dead on arrival at the Muskogee Regional Medical Center with head and internal injuries and Mann, 33, was also pronounced dead on arrival at the hospital with head and trunk internal injuries. Milligan, 51, was taken to MRMC and was later transferred by helicopter to St. Francis Hospital in Tulsa, were she was listed in critical condition with trunk and internal injuries.

The truck was equipped with seat-belts, but the passengers did not use them. Also, the vehicle had airbags, but they did not deploy.
